The order and pace of exposure is a nuanced and essential part of phobia recovery. The client should always be in charge of what exposure they practice next. Anxiety is allergic to fun. Once you can turn into your fear and do something silly, you will find the path to recovery.

Remember, the goal is not to be comfortable with vomit (people rarely ever are). The goal is to be comfortable with the possibility of vomit.
